Also, I was able to find this consent form online, which seems to be EXACTLY what you are looking for. Please understand that neither I or the ACA can warranty this form. It appears to be used by the University of Arkansas, and looks pretty good, actually 🙂

I am a grad student obtaining my master of arts degree in professional therapy. I cannot find the legal ramifications for recording a client session without the client’s prior written permission.
